Exclusive Provider Organization (EPO)

Definition
A hybrid of HMO and PPO: direct access to any in-network provider with no referrals (like a PPO), but zero out-of-network coverage except emergencies (like an HMO). Cheaper than a PPO.

EPO = PPO freedom inside the fence, HMO wall around it.
