Mandated Coverage: Diabetes and Osteoporosis
Definition
Diabetes: health policies and HMO contracts must cover ALL medically necessary equipment, supplies, and services to treat diabetes when the physician certifies medical necessity — including outpatient self-management training and education. Osteoporosis: coverage required for medically necessary diagnosis and treatment for high-risk individuals. The osteoporosis mandate does NOT reach specified accident plans, disease plans, hospital indemnity plans, Medicare supplements, long-term care insurance, or the Florida State Employee Health Program.

If the doctor certifies it's needed for diabetes, it's covered — pumps, strips, and the class that teaches you to use them. Osteoporosis testing and treatment ride along for high-risk patients.
Exam tip
Limited-benefit products (accident-only, dread disease, hospital indemnity, Medigap, LTC) are the standing exception list for many Florida mandates — learn it once.
