Wellness Rebates and Experimental Treatments

Wellness rebate: an individual health policy may rebate premium when the insured enrolls and participates in a health wellness, maintenance, or improvement program approved by the plan — based on premiums paid in the last calendar or policy year, capped at 10% of paid premiums, requiring evidence of health status via agreed indicators, and renewing annually unless the insured stops participating or fails to maintain/improve health status. Experimental treatments for terminal conditions: a plan MAY cover services tied to an investigational drug, biological product, or device — but the insurer is not required to provide coverage the Florida Insurance Code doesn't mandate.
