Point-of-Service (POS) Plan
Definition
A hybrid of an HMO and an indemnity plan: in-network care runs like an HMO (PCP + referrals + copays = highest coverage level), but the insured may go out-of-network and pay substantially more (30–40% coinsurance, higher deductibles/maximums).
In plain English
You decide at the 'point of service': use the HMO machinery, or pay extra for freedom.
