Prescription Drug Plan

Definition
Covers the cost of prescription drugs not dispensed in a hospital or extended care facility. Each plan has a formulary. Typically requires a copayment per fill. Excludes non-prescription (OTC) drugs, appliances, often hypodermic needles, third-party administration costs, and usually drugs for sexual dysfunction or infertility (unless state law requires coverage).
