Set federal minimum standards for major medical: guaranteed issue (no denial or higher premiums for pre-existing conditions), no lifetime/annual caps on essential benefits, dependent coverage to age 26, premiums ≥80% spent on care (85% for large-group plans), age rating capped at 3:1, employer mandate at 50+ full-time employees (shared responsibility payment), small-employer tax credits (<25), premium tax credits to 400% FPL, and the exchanges/marketplace.
