Must be covered with NO lifetime or annual caps: ambulatory (outpatient) services · emergency services · hospitalization · maternity & newborn care · prescription drugs · mental health & substance use treatment · rehabilitative/habilitative services & devices · laboratory services · preventive/wellness & chronic disease management · pediatric services incl. dental & vision. Preventive care is covered at 100% with no cost sharing.

PEDIATRIC dental/vision are essential; ADULT dental/vision are optional extras. A favorite trick.
