Dental Plan Types: Standalone · Integrated · Combination

Definition
Standalone plans work with any provider and keep separate deductibles and benefit limits, even inside an employer package. Integrated plans are offered with medical expense coverage and share the medical plan's network and deductible — possibly lower premiums but higher deductibles/out-of-pocket. Combination plans mix the two: their own benefit and deductible structures like standalone, but they may share a provider network; basic services paid on usual-and-customary terms while major services carry a scheduled maximum.
