Dental Treatment Levels (100/80/50)
Definition
Non-scheduled plans divide services into three levels: Preventive — cleanings, periodic exams, annual X-rays — usually covered at 100%, often no deductible. Basic — minor restorative work: fillings, extractions, root canals, gum-disease treatment — usually 80% of UCR after a deductible, up to an annual max. Major — oral surgery, crowns — deductible and annual max apply, with the insured's coinsurance typically 50%.

Exam tip
Even 100% preventive care is capped — plans limit HOW MANY cleanings/exams per year, and specialties like orthodontics carry calendar-year maximums or lifetime caps.
