Children: Adoption, Supervision, Disability and Cleft Lip/Palate
Definition
Four Florida child mandates. Adopted/foster children: covered from placement at the insured's residence; no preexisting-condition exclusion allowed — EXCEPT for a foster child. Child health supervision services: expense-incurred family policies must cover periodic physician visits from birth to age 16 (medical history, physical exam, developmental assessment and anticipatory guidance, immunizations and lab tests). Children with disabilities: coverage continues past limiting age while the child is BOTH incapable of self-sustaining employment (intellectual or physical disability) AND chiefly dependent on the employee/member for support. Cleft lip/palate (child under 18): medical, dental, speech therapy, audiology, and nutrition services when the treating physician certifies medical necessity (not required of specified accident/disease, hospital indemnity, limited-benefit disability income, or LTC policies).
