Outline of Coverage (F.S. 627.642)
Definition
Completed and sent to the applicant at the time of application, with an acknowledgment of receipt (or certificate of delivery) returned to the insurer WITH the application. Must contain: the applicable category of coverage; a brief description of the main benefits; a summary of the main exclusions, limitations, and reductions (preexisting conditions, probationary periods, limitation periods, deductibles, coinsurance, age limits/reductions); a summary of renewal and cancellation provisions including any reserved right to change premiums; a statement that it is a summary only and the issued policy controls; and, when home health care coverage is provided, a statement that such benefits are included.
