Definition
A disclosure document most states require the agent to deliver to each health insurance applicant, generally before accepting the initial premium (Florida: at application or, at latest, with the delivered policy — F.S. 627.642). For certain products such as long-term care insurance, agents must also deliver an NAIC Shopper's Guide or Buyer's Guide. (A Buyer's Guide explains health insurance generically; a policy summary describes the SPECIFIC policy, its riders, premiums, and values.)
