HIV Consent Form
Definition
Producers must secure a signed HIV consent form from the applicant and communicate that blood tests may be required. Even when a blood test is part of the insurer's regular underwriting routine, the signed consent must state that any blood drawn will be screened for HIV and that the applicant permits the testing.
