90 Days + 60 Lifetime Reserve Days

Definition
Each benefit period covers up to 90 hospital days (deductible, then days 1–60 free; days 61–90 carry a daily coinsurance = 25% of the deductible). Beyond 90 days the patient dips into 60 LIFETIME reserve days (coinsurance = 50% of the deductible) — never restored once used. After that: patient pays 100%, no cap.
Exam tip
90 days renew each benefit period; the 60 reserve days are once-per-lifetime. Blood: patient pays the first 3 pints (A and B both).
