Individual Certificates (Group Life)

A group life insurer must issue each covered member an individual certificate containing the group number and describing: the insured; the insurance protection the certificate holder is entitled to; to whom benefits are payable; any dependents covered; and the rights and conditions of the coverage.

Members don't get the policy — the employer holds that. Members get a certificate: proof of coverage, amount, and who gets paid.
