Certificate of Coverage (F.S. 627.657)

Definition
Every group health policy must provide that the insurer will furnish the policyholder, for delivery to each insured group member, a certificate showing the group number, the essential features of the coverage, and to whom benefits are payable. When dependents are covered, only one certificate per family unit is required.

The member's proof-of-membership card: one per family, not one per person.
